Oral Premedication with a New Benzodiazepine Derivative Ro 5-4200


Abstract
A new benzodiazepine derivative(Ro 5-4200) was used as a hypnotic in a. pilot study on 34 patients, having to undergo surgery. To 10 cases, a tablet of 2 mg of -the drug was given orally the night before the operation. and another 2 mg was .given again 1 hour prior to the surgery on the operative day. Rest of 24 cases, "2mg was given only on the operative day. Anesthesia was performed with _penthrane or halothane. The morning before and the day after the surgery, the patient was questioned.as to the effect of the tablet and the response was eevaluated according to the prepared questionaires.
1) It was found that with 2mg of the drug about 70 percent of patients became asleep between 20 minutes and 45 minutes and 30 percent were not asleep after one hour.
2) Patients were awoke during transportation from the ward to the operating theatre. More than 50 per cent of the patients were found sleepy or dizzy before surgery.
3) When patients fell asleep, air way tended to be obstructed because of muscle .relaxation. No complication was observed for that, but the patient should be attended by a nurse. Hallucination was observed in one case and hiccough was also observed.
4) Recovery from anesthesia was prolonged more than one hour ¢¥compared to the patients having no Ro 5-4200. Total sleeping time was judged to be 4 to 6 hours by adding the time of anesthesia, recovery and premedication.
